This proposal for The Health Trust in San Jose, California, was sent to a family foundation that didn’t have a specific application format, so I wrote a three-page piece that described the organization, its Meals on Wheels program, the need for funding, and the expected results of the desired funding.
The Health Trust applauds the work of the xxx Foundation in addressing critical community needs, particularly throughout the West Coast. The Health Trust’s Meals on Wheels program is also focused on caring for our neighbors in need. As the largest hot-meal home-delivery program in California’s Santa Clara County, last year we provided 81,903 nutritious meals to 599 homebound frail elderly and disabled adults. In addition, our drivers provide life-giving Wellness Checks and personal interaction. On behalf of The Health Trust, I invite the xxx Foundation to support this vital program with a grant of $xxx.
